【必须条件 1-5】
1. 美国硕士文凭(Master Degree), 或, 学士文凭Bachelor Degree加上2年以上的社工经验.
2. 文凭需要Psychology, Sociology, Social work, Gerontology, Healthcare, 相关专业.
3. 可以合法在美国工作
4. 懂日常英文
5. 精通囯语（交谈，閲读，中英翻译）

工作简介：

/Monitor and assess participant’s physical/mental/emotional status, provide care and support accordingly.
/psycho-social assessments for participating members.
/Meet with family members or referring agency and determine appropriate programs.
/Maintain professional attitude in critical situation。 be caring to aging / vulnerable population.
/Provides case management and coordination, include English to Chinese translation/interpretation of governmental, health care, community programs and policies.
/Complete quarterly assessment, progress notes, six-month reassessments, and initial assessment.
/Discharge planning for participating members.
/Develop monthly social support group for seniors.
/Serve as a member of the multi-disciplinary team and participate in initial assessments, care planning, and on-going re-evaluation of participant's needs.
/Process inquiries in a timely manner with tracking documentation.
/Be sensitive to environmental safety and make appropriate adjustment.
/Conduct in-home assessment as needed.
